PURPOSE: We prospectively analyzed and compared the effectiveness and complications of pneumatic lithotripter with a holmium:yttrium-aluminum-garnet (Ho:YAG) laser for the ureterorenoscopic management of impacted ureteral stones. MATERIALS AND METHODS: From January 2006 to January 2008, we performed retrograde endoscopic treatment in 288 patients with ureteral stones in our clinic. The patients with impacted stones were randomized into two groups according to the lithotripter used to fragment the stone: pneumatic (n = 40) and laser (n = 40). The preoperative, operative, and post-operative foll...

Özeti Göster

PURPOSE: We report a 2-center study of factors affecting the stone-free rate after percutaneous nephrolithotomy in horseshoe kidneys. MATERIALS AND METHODS: The postoperative stone-free rate after percutaneous nephrolithotomy was evaluated in 47 male and 11 female patients with horseshoe kidneys. All data were collected prospectively. Patient and procedure related factors predicting the stone-free rate were analyzed by univariate and multivariate tests. RESULTS: The mean ± SD stone burden was 7.62 ± 7.18 cm(2) (range 1 to 45) and the stone was larger than 10 cm(2) in 14 patients (24.1%). Compl...

Does pelvicaliceal system anatomy affect success of percutaneous nephrolithotomy?

Binbay M, Akman T, Ozgor F, Yazici O, Sari E et al.

Urology October 2011 Vol. 78(4): 733-7 DOI PubMed
Özeti Göster

OBJECTIVE: To investigate the effect of the pelvicaliceal system (PCS) anatomy on the percutaneous nephrolithotomy (PCNL) success rate. Although the caliceal anatomy is effective for stone clearance after shock wave lithotripsy and retrograde intrarenal lithotripsy, the effect of the caliceal anatomy after PCNL has not been evaluated to date. METHODS: A total of 498 patients who had undergone PCNL and preoperative intravenous urography were enrolled in our study. Kidney-related anatomic factors, such as the PCS surface area and type, degree of hydronephrosis, infundibulopelvic angle, upper-low...

Variables that influence operative time during percutaneous nephrolithotomy: an analysis of 1897 cases.

Akman T, Binbay M, Akcay M, Tekinarslan E, Kezer C et al.

J Endourol August 2011 Vol. 25(8): 1269-73 DOI PubMed
Özeti Göster

BACKGROUND AND PURPOSE: Operative time is an important perioperative factor and is related to postoperative complications and procedural cost-effectiveness. There are few studies, however, investigating factors that affect operative time during percutaneous nephrolithotomy (PCNL). In this study, patient and kidney stone-related factors that influence operative time were analyzed. MATERIALS AND METHODS: Clinical records from 1897 patients with renal calculi who underwent PCNL were retrospectively reviewed, and these patients were divided into two groups based on their median operative time (gro...

Outcomes of percutaneous nephrolithotomy in patients with solitary kidneys: a single-center experience.

Akman T, Binbay M, Tekinarslan E, Ozkuvanci U, Kezer C et al.

Urology August 2011 Vol. 78(2): 272-6 DOI PubMed
Özeti Göster

OBJECTIVES: To evaluate the outcomes, complications, and early and late postoperative kidney function of percutaneous nephrolithotomy (PCNL) in patients with solitary kidneys. METHODS: Between 2002 and 2009, 47 patients with a solitary kidney (congenital in 10 patients, 21.3%; contralateral nephrectomy in 22 patients, 46.8%; and nonfunctional kidney in 15 patients, 31.9%) underwent PCNL. Serum creatinine was measured preoperatively, on postoperative day 1, and at each follow-up visit at regular intervals. The 4-variable modification of diet in renal disease equation was used to calculate the e...

Updated epidemiologic study of urolithiasis in Turkey. I: Changing characteristics of urolithiasis.

Muslumanoglu AY, Binbay M, Yuruk E, Akman T, Tepeler A et al.

Urol Res August 2011 Vol. 39(4): 309-14 DOI PubMed
Özeti Göster

The worldwide prevalence and epidemiologic characteristics of urolithiasis appear to have changed in the last decade. This study aims to update the current understanding of the disease in Turkey. A representative sample, totalling 2,468 participants between 18 and 70 years of age from 33 Turkish provinces, was enrolled in this cross-sectional study conducted with a professional market investigation company. Participants were evaluated with face-to-face interviews by medical students using a standard questionnaire. Of the 2,468 participants, 274 (11.1%) reported a history of urinary stone disea...

Clinically insignificant residual fragments after percutaneous nephrolithotomy: medium-term follow-up.

Altunrende F, Tefekli A, Stein RJ, Autorino R, Yuruk E et al.

J Endourol June 2011 Vol. 25(6): 941-5 DOI PubMed
Özeti Göster

BACKGROUND AND PURPOSE: Clinically insignificant residual fragments (CIRFs), defined as asymptomatic, noninfectious, ≤4  mm fragments, are sometimes observed after percutaneous nephrolithotomy (PCNL). Because the natural history of these fragments is unclear, we investigated the medium-term outcome of these fragments. PATIENTS AND METHODS: During a 3-year period, 430 patients underwent PCNL. Overall stone-free rate was 74.5%, and CIRFs were encountered in 22% of cases 3 months after surgery. A total of 38 patients who had CIRFs immediately after PCNL with at least 24 months of follow-up were i...
